Health Card Version Code & Expiry Date Update Pop-Up

Requirement for Ontario Patients to Update their Health Card Information before Booking

Updated 4 days ago

Important Reminder: The "Renew Date" in your EMR is what triggers the health card update pop-up.

Two Scenarios:

  1. A patient’s Health Card “Renew Date” has passed.

  2. A patient’s health card “Renew Date” is within 3 months.

A patient’s Health Card “Renew Date” has passed:

  1. When a patient attempts to book an appointment, they will enter their health card number and date of birth

2. After clicking "Proceed to Booking," a pop-up will appear prompting them that their health card is out of date, and they will need to enter their new version code and expiry date.

Please note: "New Effective Date" refers to the start date, and "New Expiry Date" indicates when the health card will need to be renewed again.

A patient’s health card “Renew Date” is within 3 months:

  1. When a patient attempts to book an appointment, they will enter their health card number and date of birth.

  2. After clicking "Proceed to Booking," a pop-up will appear prompting them that their health card is about to expire and they will need to update their health card information.

Please note: If the patient does not yet have their new card, they can click on "I don't have my new card yet" and proceed with booking their appointment normally.
