Updates to appointment booking:

1. Moved the terms of service to the first step of booking and clarified policies for Cortico vs the clinic into two separate documents.

2. We now allow booking in-clinic appointments to show available doctors across multiple locations when booking from a special link.

3. "Remember Me" now saves the email address in step 1 for patients booking without a Health Card Number

Updates to the Clinic Settings page:

1. Added FTE status (Inactive, Non-billed, 0.5 FTE or 1 FTE) in the Staff tab.

2. Visual bugfixes to the Workflows tab on mobile devices.
