Added a setting to include the patient agreement text in the appointment confirmation email.

In the Virtual Appointments page, the "Send video link to patient" feature now sends the Doxy.me link instead of the Cortico video appointment link if the doctor has a Doxy link set up.

Added a "Job Title" field for doctors, with the options "Doctor", "Nurse", and "Counsellor". This allows us to customize how the doctor/provider's title is displayed instead of always displaying "Doctor".
