Updates to the appointment booking page:

1. When booking a family doctor appointment with a doctor who works at multiple locations, the selected location will be shown during the booking process.

2. For clinics with multiple locations, show the location's email address in the error message if appointment booking fails, instead of the main clinic's email address.

3. Added first available time to "Available today" and "Available tomorrow" statuses in the doctor dropdown list. (e.g. "Available today from 4PM", "Available tomorrow from 9AM")

Updates to the clinic settings page:

1. The welcome email (for new doctors) is now separate from the password reset email, and contains additional information and context for doctors to get started.

2. Changed the "Send welcome email" button to "Send password reset".
