(Beta) Continued work and bugfixes on the OpenEMR integration.

Updates to the new patient form:

1. Fixed a bug where pressing the "enter" key on the keyboard did not submit the form.

Updates to the Provider Schedule page:

1. Added "Send SMS reminder to patient" feature for all appointments. (Previously, only video appointments had this option available.)

Updates to the Patient Portal:

1. The popup warning when confirming or cancelling appointments is now styled to be consistent with Cortico branding.

2. Other visual cleanup and fixes.

Updates to the Confirm/Cancel appointments page:

1. Updated the "Finish" button to link to the clinic website instead of the Cortico homepage.

Updates to appointment booking:

1. Performance improvements on booking page.

2. We now save some encrypted appointment data (e.g. selected workflow) to help with Cortico features that don't have a corresponding field in Oscar.

3. Reason for visit dropdown options are now sorted alphabetically instead of by order added.

Updates to Clinic Settings page:

1. Added the (general, not workflow-specific) email policy field.
