Updates to appointment booking:

1. Added one-click links to confirm or cancel appointments in the confirmation and reminder emails sent to the patient.

2. We now show exact days (e.g. "Available in 3 days") in the doctor selection dropdown, instead of "Available within 7 days", and sort this list by earliest availability.

3. The Reason for Visit field has been moved above Diagnostic Questions in workflows that use both features.

Updates to the new patient form:

1. Added a warning if the patient selects a province that is different from the clinic's province.

Updates to terminal booking:

1. Fixed a bug where the patient agreement was not being rendered in HTML.
