Store patient's phone number in the Reason for Visit (Appointment Notes) field in Oscar.

Updated the error text in the appointment booking page that appears when a patient has entered incorrect info when looking up their profile.

Added a message in the appointment booking page that appears if the earliest appointment is more than a week in the future that suggests checking a locum's availability (if applicable), another doctor's availability (if applicable), or other booking options (via the Drop In page; [clinic].cortico.ca/drop-in).

Bugfix: Only show booking option header (e.g. "Specialists") if there are booking options in that category.
