1. Updates to Request to Book:

1.1 - Performance improvements implemented for the Request to Book table.

1.2 - If there is only one "reason for visit" when creating a new invite, it will be selected automatically.

1.3 - If there is only one medium when creating a new invite, it will be selected automatically. "Not configured" options will no longer appear in the medium dropdown.

3.3 - Fixed a bug where, when the provider sends an initial message to the patient, the patient would receive two email notifications instead of one. Also, when patients replied to this message, it would mistakenly send them another email notification.
