Updated email body content to include video appointment instructions when an MOA uses the "Email link to patient" button on the Video Appointments portal page.

Added a different error message if the patient is a new patient but the workflow they are booking through doesn't accept new patients.

Added an option to have a different appointment status code for private-paying patients. This can be used to flag private-paying patients a different colour in the doctor schedule in Oscar. This appointment status code will be used instead of the appointment medium status code for private-paying patients.
