Updates to the appointment booking page:

1. The appointment times have been changed from 24H format to 12H format (e.g. "1:00 PM" instead of the previous "13:00 PM").

2. Added a new error message for new patients who try to book appointments for services that don't take new patients and a new page (/patient-signup) that lists services that new patients can book appointments for.

Updates to the terminal booking page:

1. We no longer show the Reason for Visit step if a patient is checking in to an appointment they've already booked.

Updates to the Clinic Settings page:

1. Added a field to configure the template code an appointment is set to after a reminder email is sent to the patient ("Emailed Appointment Status").

2. Added input validation to the email fields and provider number field.

Updated several pages to use the new menu and navigation bar.

[backend] The provider number is now a required field for Doctors.
