(Beta) Specialist booking:

1. Added a new page for staff to triage patients for specialists and generate temporary, single-use booking links.

2. Patients can only book with specialists by invite from the clinic (i.e. it will not be publicly available from the Book page)

3. Appointments can be specified as "New referral" or "Follow-up", the booking range can be customized (e.g. patients can only book an appointment between Nov 10 - Dec 10), and the expiry date of the generated booking link can also be customized.

Usability improvements to the Patient Portal:

1. Added text "(if applicable)" to photo upload step to make it more clear that it's optional.

2. Show diagnostic forms (but not the submitted answers, for security/privacy reasons) even after they've been submitted.
