Send email notification to clinic when patient cancels their appointment.

Updated Reminder Reports sorting to group records by demographic number. 

Fixed "Available today" not showing up in workflow card with same day booking range.

Included cancelled appointment timeslot backed to cached available timeslots.

Updates to Diagnostic Forms:

1. Updated wording for error message in diagnostic forms.

2. Fixed issue with two forms assigned to the same workflow causing forms to repeat indefinitely.

3. Visual improvements for long forms and/or multiple forms. (postponed to next release)
   

Updates to Book by Invite/Specialist Booking:

1. Removed confirmation alert when submitting invite.

2. Added autodetecting values in querystring to autopopulate fields when creating an invite.
