Updates to Diagnostic Forms:

1. Added diagnostic forms for new patients signing up.

2. Included new configuration for diagnostic answers to include a message when answer is selected, checking if selected answer terminates booking, and service fee.

Added preset reason field when creating a book by invite. When patient books an appointment, if the preset reason is set, it will auto-fill the reason for visit.

If patient is using two-factor authentication yet unsubscribed, patient will be resubscribed to receive emails or SMS.

For private patients signing up, state/province will only set province and exclude health card type in EMR records.

For Oscar Pro, included a fix for "Consent to use Email for care" being overriden when patient books an appointment.
