Patient Forms Updates

1. When "Is form publicly accessible" is enabled, the selected form will be available for only existing patients to fill out. Patients don't need an appointment to fill out the form.

This can be enabled in the form editor (https://clinicname.cortico.ca/patient-forms) 

2. When a form is filled out, clinics can separate configurations for generating a PDF of form results and sending an email notification.
