Patient Portal now moved out of alpha feature

Update to appointment booking:

1. In step 3, when checking for duplicate appointments, we no longer include current date for checking future appointments.

Update to confirm/cancel appointments page:

1. If the selected appointment's date is more than 8 days after, the "Confirm Appointment" button should be disabled and a message should appear saying, "Appointments can only be confirmed within 8 days prior to the appointment date."

Updates to locations:

1. Added field to store Google reviews URL based on location.

2. Location slug will now be based on Oscar location set when creating a new location in the clinic settings page.

Included new workflow events for successful and failed SMS reminders.
