Improvements to the doctor waiting time estimate in video appointments, and displaying the text "The doctor is expected shortly" if we don't have enough information (i.e. non-video appointments in their schedule) to provide a time estimate.

Updates to the Diagnostic Questions feature:

1. We now allow questions to have freeform text answers.

2. Diagnostic results longer than 50 characters will be exclusively sent by email and will be noted in Oscar reason field as [Diagnostic details emailed]
