Upon finishing signing up for new patient, patient will now be directed to Step 2 in appointment booking.

Included fixes for duplicate provider numbers for multiple Cortico providers in different locations.

Updates for Emails:

1. Added more information in payment confirmation email such as patient's first and last name, which doctor the patient paid to, date and time of the appointment, and selected appointment medium (in-clinic, video, phone).

2. Fixed email text for phone appointments.
