Updates to the video appointments list page (for staff):

1. Fixed a bug where trailing whitespaces in the Appointment Resources field (e.g. "virtual " or "phone ") were causing the appointment to not be recognized as that medium.

Updates to video appointments:

1. Fixed a bug where doctor names weren't being displayed.

2. Added an error page that is shown if an invalid appointment URL is accessed.

Updates to Clinic Settings:

1. In the Staff tab, deactivating a user will also deactivate any providers associated with them.
